Open Enrollment Class Certification Cards Certification Requirements:  Students must successfully complete entire course session including all practical skills and/or written testing requirements for certification purposes. Instructor reserves the right to fail/dismiss any students who do not fully meet course requirements or disrupts classroom instruction in any manner including but not limited to unethical or harassing conduct. No refunds shall be issued for any students who fail, do not complete, or are dismissed from a course for any reason. *Note: Minimum age for certification 11 yrs and up. Certification Equivalence:  American Heart Association (AHA) or American Safety & Health Institute (ASHI) certificates may be issued depending on the course selected. All Code Blue programs adhere to the latest nationally approved standards for CPR & ECC (2005) Guidelines, and are equivalent to American Red Cross® certifications. Both AHA & ASHI programs are approved by OSHA, DOT, JCAHO, NREMT, CECBEMS, EMSA, LACo.EMS, LACo.DCFS & CA Teacher Credentialling. *Note: Customer is solely responsible for ensuring course will meet specific school or workplace requirements.   Certification Cards:  Registered students will generally be issued course certification card(s) on-site at class, or within 3-5 business days following course completion. If student needs proof of course completion before certificates are issued, they may contact training@codeblueonline.com to request a letter of verification as proof of course completion/certification.   Replacement Cards:  Code Blue is not liable upon the relinquishment of certification card(s) to student in the event they are lost, stolen, damaged, or destroyed; and/or for misspelled names due to student's illegible writing on sign-in sheet or registrant's error on registration form. A $10 replacement fee (per card) shall be payable in advance of the issuance of duplicate certificates if requested. © 2010 Code Blue Emergency Response, LLC All rights reserved.
